1. Non-medical (for ages 0 to 14, APS may be required)

* For Accelerated Health Care of less of $20,000, blood profile replaced by urine.
** For 15 to 17, urine is also required.
 For amounts ranging between $500,001 and $999,999 the appropriate financial questionnaire is required (not applicable to Executive Savings Plan nor to insured aged 17 or less).

Notes:
Sum insured refers to the total amount of insurance (critical illness and accelerated health care) applied for within the last 6 months at DFS.
Medical requirements are valid for a period of up to 6 months.
PSA as of age 51 and $0 for men.
